Incident Escalation

Incident escalation refers to the process of routing an AI-related problem, such as a harmful model output, a data exposure, or a system failure, up the chain to people with the authority to act on it. For enterprise AI governance, it defines who must be told, how quickly, and what decisions they own, which matters because regulators and courts increasingly ask whether organizations responded to known AI harms in a timely and documented way. Clear escalation paths also help legal, compliance, and executive teams decide on disclosure, customer notification, or a pause in deployment before a minor issue becomes a public failure.
