Security Incident

A security incident is an unauthorized access, breach, or attack on systems, data, or infrastructure that compromises confidentiality, integrity, or availability. For AI governance, security incidents involving AI systems—such as unauthorized model access, training data theft, or prompt injection attacks that manipulate AI outputs—create compliance obligations, regulatory reporting deadlines, and potential liability. Organizations must establish incident response procedures, document what happened, notify affected parties where required, and demonstrate to regulators and auditors that they controlled the damage and prevented recurrence.
